Making appointments for consultations or treatments is easy with Guymon Physical Therapy & Wellness. You can either give us a call at the location nearest you or click on the "Book Appointment" button at the top of the page to have our office professionals give you a call back.

Pre-Visitation

To make an appointment, simply call our Guymon Clinic at 580.338.4789 or our Liberal Clinic at 620.629.6390 to get scheduled right away or click “Book Appointment” at the top of the page.

Before your first visit, we recommend that you:
  • Make a list of questions that you would like to discuss on your first visit.
  • List signs and symptoms that you believe could be of importance to our physical therapist.
  • Write down your past medical history, previous treatments, medications your taking or have taken in the last 60 days and any other information you deem important.
  • Contact our offices to verify your insurance benefits are intact.
  • Print and complete our patient forms below as fully as possible before your scheduled visit.
  • Show up 15 minutes early for your first appointment to make sure we have everything in order.
  • Bring your insurance card and a photo ID.
  • Book your appointment as far ahead as possible to avoid congestion during the busiest parts of the day.

Expectations of Your First Visit

During your initial evaluation your physical therapist will ask questions about your medical history, current pain, cause of your current injury, and other details that may be of importance.

A general evaluation of your present health, movement, strength and coordination will be conducted. Our ultimate aim is to help you get better faster so be sure to communicate clearly if pain or discomfort presents itself during the initial evaluation.

Once your primary evaluation is complete, your physical therapist will recommend and prepare a course of action with treatments and/or exercises to minimize your recovery time while maximizing results.

There will likely be some physical treatments performed on the first day to initiate your road to recovery. Additional, and more intense treatments, may ensue on subsequent visits. How much physical therapy you will require will be discussed based on your initial evaluation and/or performance of initial treatments.

Before you leave, we'll likely ask that you schedule your series of visits with the front desk per your physical therapist. The number of visits will be discussed before, during or after treatments and may vary depending on your progress. To avoid inconveniences in your schedule, book your appointments as far ahead as possible.

Rest assured that your therapists will chart all findings and treatment plans to submit to referring physicians or other medical professionals should you require it.

Insurance Information

At Guymon Physical Therapy & Wellness, we accept the insurance plans listed below in addition to workers' compensation plans. If you don't see your plan or provider, give us a call as we are always updating insurance plans. As an added courtesy, our friendly staff will verify your health benefits before you arrive or during scheduling to determine if you will be responsible for a copay, deductible, or co-insurance for any physical therapy services we perform.

Should you happen to not have health insurance or have maxed out your benefits, we offer a private pay plan. Please give us a call for more information on our service rates and payment details.

  • What is Physical Therapy?

    Physical therapy is a medical, hands-on approach to alleviating pain and restoring proper function to your body through various treatments and/or exercises. Our physical therapists are trained experts in musculoskeletal and neurological conditions and work closely with other medical physicians. Our physical therapists evaluate your movement patterns, identify the cause of your pain, and work to minimize or eliminate it completely.


    Because every patient’s condition and pain may be unique, our physical therapist will employ the necessary gentle hands-on techniques, exercises, and equipment to alleviate pain and to restore proper function to your body. Our overall goal is to restore pain-free mobility and function while minimizing the chance of re-injury or reemerging pain.

  • Is Physical Therapy Right For Me?

    We really require a little more information before determining if physical therapy is the right route in treating your condition and/or pain. We will work closely with your physician to investigate all possible options and make a determination based on that data.   • Is Physical Therapy Painful?

    The ultimate aim of physical therapy is to alleviate pain. Unfortunately, in some instances, restoring your mobility may lead to some discomfort, especially in the early stages of treatment, but can fluctuate depending on the condition(s) being treated. Soreness is common in cases where muscles have atrophied or are recovering.


    Should you ever experience any increase in pain, whether during or after treatments, please communicate it immediately to your physical therapist. We shoot for the quickest recovery to get you pain-free and back to normal fast.

  • How Do I Book An Appointment?

    We make booking appointments super easy. Simply give us a call at the location of your choice, and our friendly office personnel will schedule you at a time that is convenient for you. We do offer morning and afternoon appointments to best accommodate those with busy lifestyles but still recommend booking as far in advance as possible to secure your spot during prime times.

  • What Should I Bring to My First Appointment?

    If you have your prescription and insurance cards available when you call, we can verify your insurance benefits and provide you with an estimate of coverage in a matter of minutes.


    First Visit Checklist:

    • A prescription, if referred, for physical therapy from your referring physician, ARNP, or PA.
    • Insurance Information / Insurance Card
    • Printed and filled out patient forms. If you're unsure which ones you need, give us a call.
    • In the event of an automobile accident or worker’s compensation claim, bring any case manager or insurance adjuster contact information including his/her name, contact phone numbers, and claim number.
    • Show up 15 minutes early to complete any outstanding paperwork allowing you maximum time for your evaluation.
    • Wear comfortable, loose-fitting clothing that allows easy access and full range-of-motion to your problem area(s).
  • Are My Treatments Covered by Insurance?

    The short answer: Yes, your insurance provider typically covers physical therapy treatments. To verify coverage, give the office of your choice a call, and our friendly office personnel will contact your insurance provider to help you determine your coverage.


    Should your insurance carrier not cover the planned treatments, we do offer flexible self-pay options to ensure you get the medical treatments you need and deserve.

  • What If I Don't Have Insurance?

    If you don't carry insurance or have maxed out your benefits, we do offer flexible self-pay options that typically allow you to pay as you go. Get with our offices for more information or to verify insurance options available to you.

  • What Do You Recommend I Wear?

    We prefer that you dress comfortably with clothes that allow easy access and full range-of-motion to the problem area(s) to allow our physical therapist to examine you fully. Athletic shoes are often the best choice dependent on the necessary treatments. We also advise you to bring hair ties in case you want to pull your hair up during treatments.

  • How Long Do Therapy Sessions Last?

    Typically, initial sessions lasts from 60 to 75 minutes allowing for detailed explanations and full evaluation of your problem area(s). Subsequent sessions run from 30 to 45 minutes depending on progress and a few other variables. These are estimates; actual times will be based on your specific needs which will be discussed with you by your physical therapist.

  • How Many Sessions Will I Need to Attend?

    We will require additional details in order to determine the extent of physical therapy you require. We will collaborate with your referring physician, when necessary, and create a plan-of-action on your initial visit to give you an anticipated number of sessions to get your life back on track.

  • What Qualifications & Credentials Do Your Physical Therapists Have?

    Physical therapists must have years of collegiate-level training in anatomy, physiology, kinesiology, neurology and many of the other medical sciences. Our physical therapists have graduated from accredited physical therapy programs and are properly licensed and/or certified in their states of practice. Our physical therapists possess a minimum of a masters degree and have years of experience practicing under other licensed physical therapists.


    Rest assured that we know what we're doing when it comes to alleviating bodily pains and discomforts to maximize your quality of life.

  • Do You Offer Maintenance Programs Even After I'm Cleared?

    Yes! In fact, we recommend that you continue a maintenance program once you've completed all recommended physical therapy sessions under our supervision. We will create a home-exercise program for you to help you to help you maintain, and in some instances, increase your level of function and comfort.

  • Do I Have Direct Access to Physical Therapy Services in My State?

    In both Kansas and Oklahoma, Physical Therapists are accessible to you by direct access, meaning you do not have to wait for a physician's referral in order to obtain services. Direct Access is a huge benefit to you in that if you have pain or an injury that you believe could be remedied by physical therapy services, you do not have to wait to see a doctor and then wait to get in to see the clinicians at Guymon Physical Therapy and Wellness. In most cases, the sooner you begin treatments, the better your overall recovery will be and the greater the chance of full recovery.
